Bile Peritonitis





Abstract
Bile peritonitis is known as a relatively rare condition. Recently, in our institution, the incidence seems to be increasing probably due to increased number of the abdominal trauma patients, diagnostic procedures such as those of needle biopsy of the liver, Percutaneous Transh-epatic Cholangiogram in jaundice, and increased number of the patients in biliary surgery.
This report deals with 27 patients of bile peritonitis which was confirmed at operation or paracentesis during the last10 year 8 months period from August 1965 through April 1976.
Twenty-five patients underwent operation with 2 deaths and unoperated 2 patients were died.
Fourteen of the patients were male, and the average age was 48 years. The causes of the -bile peritonitis in our series were spontaneous perforation of the diseased gallbladder in 6 patients, after operation on or, near biliary tract in 7 patients, after liver biopsy in 3 patients; after P.T.C. in 3 patients, and after-blunt abdominal trauma in 4 patients. Another 2 cases were spontaneous perforation of C.B.D. and choledochal cyst in infancy.
The choice of operative treatment were simple drainage, primary closure of the leakage site with drainage, and definite surgery of the primary disease in biliary tract in our series. In our series, the prognosis were depended upon the presence of infected bile.
